Inevitably, there will be mistakes and some ideas that are not 100% clear in any book of this nature.  Here we try to keep a record of corrections and clarifications for the reader.  Please, if you have anything to contribute, please do so via PM or in the Page by Page section (where questions are asked and addressed on an individual page basis).

Corrections in this section have page numbers, and are designed to print out such that they can be placed (or glued) into your book.  The numbers right of the page number deciml point on the page numbers will indicate between which two pages of your book the correction should be inserted.
